OSHA 1926.502(k) requires a written plan when conventional fall protection is infeasible. But most GCs require a written plan for ALL elevated work regardless. Without documentation of your hazard assessment, system selection rationale, and rescue procedures, you have no defense when OSHA arrives after an incident.
OSHA 1926.502(d)(20) requires prompt rescue after a fall arrest event. Suspension trauma can become life-threatening within 15–30 minutes. If your only plan is “call 911,” that’s not a rescue plan — that’s a hope. You need designated rescuers, pre-rigged equipment, and a documented response time target.

Follow this sequence before any work begins above 6 feet. Your competent person leads each step.
Walk every work area where employees will be exposed to fall hazards. Identify all unprotected sides, edges, holes, and elevated surfaces 6 feet or more above a lower level. Document each hazard location on the assessment form.
For each identified hazard area, your competent person selects the appropriate fall protection system following OSHA’s hierarchy: elimination first, then prevention (guardrails), then fall arrest (PFAS), then administrative controls (warning lines + monitors).
Never allow a worker to tie off without a rescue plan. Suspension trauma can incapacitate within minutes. Your rescue plan must be specific, practiced, and capable of retrieving a suspended worker within 15 minutes.
Before any worker ties off, the competent person inspects every component of the fall protection system. Document the inspection even when everything passes — the log proves your diligence after an incident.
