Confined Space Incidents Are Fast, Deadly, and Preventable

Confined space accidents kill workers in minutes. Atmospheric hazards — toxic gases, oxygen deficiency, explosive vapors — can incapacitate a worker before they can self-rescue. OSHA's permit-required space standard exists because these incidents keep happening.

The Permit System Must Be Written

OSHA requires a written permit-required confined space program — not just a verbal checklist. The program must name the spaces, define the evaluation criteria, describe the entry procedures, and specify the rescue arrangements. An unwritten program doesn't satisfy 1910.146(e).

You can't smell or see oxygen deficiency, methane, hydrogen sulfide, or CO until it's too late. Atmospheric testing before entry and continuous monitoring during work is the only defense — and OSHA expects documented proof of test results on the entry permit.

Calling 911 after a worker collapses in a manhole is not a rescue plan. OSHA requires that rescue services be identified and available before entry begins. The rescue plan must specify who is coming, how they get there, and what equipment they use.

Know Which Spaces Require a Permit

Not every confined space requires a permit. But every space must be evaluated. This program includes the evaluation form your competent person needs to make that determination correctly.

Non-Permit Confined Space

A confined space that does not contain or have the potential to contain any hazard capable of causing death or serious physical harm. Must still be evaluated by a competent person before entry.

Contains or has the potential to contain a hazardous atmosphere OR other recognized serious hazard. Requires the full written program, formal entry permit system, attendant, and rescue provisions.

The Four Atmospheric Hazards You Must Test For

Atmospheric testing before and during every PRCS entry. This template includes the atmospheric testing log with all four hazard categories. Each test must be documented on the entry permit.

Oxygen Deficiency / Enrichment

19.5% – 23.5%

Oxygen must remain within this range. Below 19.5% causes impaired judgment and loss of consciousness. Above 23.5% creates fire risk — materials that don't burn in normal air will ignite.

Flammable Gases & Vapors

Below 10% LEL

At 10–25% LEL, work may continue with continuous monitoring. Above 25% LEL, entry must be halted until the space is ventilated and retested. Confined spaces accumulate flammable gases from decomposition, fuel storage, or adjacent work.

Above 25% LEL = NO ENTRY

Hydrogen Sulfide (H2S)

PEL: 10 ppm

H2S is common in sewers, manholes, septic tanks, and excavations with organic material. Smell dissipates quickly at high concentrations — don't rely on odor detection. Rapid incapacitation at high levels. IDLH: 50 ppm.

Carbon Monoxide (CO)

PEL: 50 ppm (8-hr TWA)

CO is produced by combustion engines, gas-powered equipment, heating systems, and hot work operations in or near the space. Colorless, odorless — requires direct monitoring. IDLH: 1,200 ppm.

Stratification Risk

Test top + mid + bottom

Atmospheric conditions are not uniform in confined spaces. Methane and CO2 are heavier than air — they settle to the bottom. Lighter gases accumulate at the top. Test at multiple points before declaring a space safe.

Atmospheric conditions can change during work — equipment operating inside the space, ventilation failure, groundwater intrusion, heat buildup. Monitoring must continue throughout the entry, not just at the start.
