The 4 Trenching Mistakes That Trigger OSHA Citations on Houston Job Sites

OSHA 1926.652(a)(1) requires a protective system for any excavation 5 feet or deeper — sloping, shoring, or shielding. Unprotected trenches are the most common Subpart P violation and almost always classified serious or willful when a cave-in fatality occurs.

Guessing the soil type without performing required visual and manual tests (OSHA Appendix B) means your sloping ratios may be wrong. Spoil Pile Too Close to Edge

Excavated material placed within 2 feet of the trench edge creates a surcharge load that increases lateral pressure on trench walls and dramatically raises cave-in risk. OSHA Appendix B — Three Soil Types, Three Sloping Ratios

Your competent person must classify the soil using at least one visual test and one manual test before selecting a protective system. Classification determines everything — use the wrong type and your sloping calculation is wrong.

Pre-Excavation: Locate Underground Utilities

Call 811 at least 72 hours before digging (Texas law). Document the notification, wait for marking, then pothole to physically verify locations. Never assume utility maps are accurate in Houston's older neighborhoods and industrial corridors.

The competent person must inspect the excavation before each shift begins and after any event that could have increased hazards — rain, frost, vibration, or adjacent work. Document every inspection even if no hazards are found.

Emergency Response — Cave-In Protocol

If a cave-in occurs or is imminent, do not send rescuers into the excavation without mechanical means of rescue. Entering an unstable trench after a collapse is how single fatalities become double fatalities. Your plan must address non-entry rescue first.

  • Evacuate all workers from the excavation immediately — do not attempt entry rescue
  • Call 911 and provide trench location, depth, number of workers potentially buried
  • Secure the area — prevent additional workers or bystanders from entering
  • Use rescue equipment (rope rescue, mechanical lift) for non-entry extraction where possible
  • Cooperate with emergency services and do not disturb the collapse site
  • Report to OSHA within 24 hours of any in-patient hospitalization or fatality

For general industry maintenance work that involves excavation, OSHA may also apply general industry standards — but on construction sites, Subpart P controls. OSHA 1926.652 requires one of three protective systems for excavations 5 feet deep or more: Sloping — cutting trench walls back to a stable angle based on soil classification (Type A: 3/4:1; Type B: 1:1; Type C: 1.5:1). Shoring — installing timber or hydraulic supports to brace trench walls. Timber shoring specifications are in Appendix C; aluminum hydraulic shoring in Appendix D. Required when sloping is impractical due to adjacent structures or space constraints. Shielding (trench boxes) — placing a prefabricated steel or aluminum shield inside the trench. Workers must stay within the shield at all times. The shield must be rated for the soil pressure at the trench depth.
