Common questions about confined space permits

Under OSHA 29 CFR 1910.146, a confined space has three characteristics: it is large enough for a person to bodily enter and perform work; it has limited or restricted means of entry or exit; and it is not designed for continuous occupancy. Common examples include manholes, tanks, vessels, silos, storage bins, hoppers, vaults, pits, pipes, and crawl spaces. A permit-required confined space additionally has one or more serious hazards — a hazardous atmosphere, engulfment potential, internal configuration hazards, or any other recognized serious safety or health hazard. Permit spaces require a written entry permit before entry.
A written entry permit is required under OSHA 29 CFR 1910.146 before any authorized entrant enters a permit-required confined space. The permit must document the space being entered, identify all hazards, list hazard control measures, specify acceptable entry conditions (atmospheric readings), list authorized entrants and attendants, identify the entry supervisor, and identify rescue and emergency services. A new permit is required for each entry, and completed permits must be retained for at least one year.
OSHA 1910.146 requires atmospheric testing for oxygen content, flammable gases and vapors, and potential toxic air contaminants before and continuously during entry. Acceptable entry conditions are: oxygen content between 19.5% and 23.5%; flammable gas or vapor below 10% of the lower explosive limit (LEL); and toxic contaminants below OSHA permissible exposure limits (PELs). Common toxic gases in confined spaces include carbon monoxide (CO), hydrogen sulfide (H₂S), and nitrogen oxides. Testing must be performed by a qualified person using calibrated instruments, and all results must be recorded on the entry permit before authorization.
The entry supervisor is a knowledgeable person — typically a qualified safety officer, foreman, or experienced supervisor — who understands the hazards in the permit space and the means to control them. Under OSHA 1910.146, the entry supervisor is responsible for verifying that all required measures listed on the permit have been completed before authorizing entry, ensuring rescue services are available and able to respond, authorizing entry by signing the permit, overseeing entry operations, and canceling the permit if conditions change. The entry supervisor may also serve as the outside attendant, provided they remain outside the space and maintain continuous communication with entrants.
